Relmyna Verenim is back at Xaselm and continuing with her experiments. Nanette Don can also be found there; now that Passwall has been invaded and retaken, she feels safer at Xaselm, although one wonders why. You need to go there and talk to Relmyna about a new heart for Syl. You can go in the back door this time.
You actually need two things, the new heart, and Relmyna's assistance in installing it. She has a heart from one of her experimental subjects that she thinks will be appropriate. The heart is strong, although the rest of the body was badly damaged (by Relmyna, directly or indirectly).
In return, Relmyna wants you to find a special item for her. The Tentacle of Intrusion is at a small ruin in the south of the Isles, not far from Corpserot Prison. There are a number of leveled undead guarding the entrance, and more in the partially flooded ruin beneath. There are several regular swamp tentacles growing down there, but the one you're looking for is in an urn at the end of the root tunnel.
