The unmarked Fort Dementia becomes the Old Fort with the addition of an entrance to an underground ruin. Relmyna send you here to find the Tentacle of Intrusion.

Entering the ruin, you descend a flight of stairs and there is a cross-passage in front of you. Going left, you pass over a bridge to a dead end with two leveled undead and an urn. Right takes you to a gate which is opened by the pushbotton on the wall next to it. A flight of steps leads down to a partly flooded hall with more undead, and a number of swamp tentacles and fungus stalks. There are chests at either end.
There is a tunnel entrance above you, but it cannot be reach from this end. Going back to the entrance, you can take another passage into a larger hall with two more undead, and a pair of Hunger Statue traps. At the far end is a gate that leads to an urn and a root tunnel. There are three more undead in the tunnel, which leads to the small hall.
Just before you drop into the hall, there is an urn containing the Tentacle of Intrusion.
Sheogorath's Retreat adds a trapdoor to the previously unmarked Fort Mania, which leads to a subterranean Pleasure Dome.
Scrying the Gate: Locate the Mania gate to Oblivion
There are no enemies in this area, only Sheogorath's Playmates (one Mazken and one Aureal) and their pet Hunger, Slim. In the outer corridor, there are several busts of Sheogorath and a smaller version of the Font of Madness.
A chest is behind the bed, but does not contain any significant treasure.
The Nursery of Delerium is entered from the teleport pad leading out of the Pool of Reflection. That area is a clone of the original, without the doppelganger, so this area is added to provide some challenge. It is populated with Gnarls and Hungers.
Scrying the Gate: Locate the Mania gate to Oblivion
This root tunnel section is modified from an unused one made by Bethesda (as a test?) for the Fountainhead. It contains several Gnarls, and two Hungers at each of the small pools. It is possible to avoid most of the enemies in this section by following the north passage.
There is an Amber stump at the southern side of the large cavern in the middle, and a few hollowed stumps scattered around.
